To help ensure a smooth visit, please complete the following steps before your appointment:
- ✔ Complete the New Patient Forms, print them, and bring them with you.
✔ Bring your insurance cards and a valid photo ID to your appointment.
✔ Arrive 15 minutes before your scheduled appointment to allow time for check-in.
✔ Bring a complete list of your medications, including prescribed medications, over-the-counter drugs, vitamins, and supplements.
✔ Be prepared to make any necessary co-payments and/or co-insurance at the time of your visit. We accept checks, credit, and debit cards (Visa, MasterCard, and American Express). If you are unable to make a payment at the time of service, please contact our office in advance to arrange financial accommodations.
